Referrals to the IMCA service in Hampshire

Referrals to the service can be made by completing the form below and returning it to us. Alternatively, you can contact us to discuss it in more detail, or use the "Do you need an IMCA" checklist to make sure the referral is eligible for an IMCA. You should refer all clients and patients who are currently residing in accommodation or hospitals in Hampshire, regardless of where they ordinarily reside.

All referrals will be acknowledged within 1 working day by the IMCA co-ordinator, who may need to discuss the case with you in more detail. The case will be allocated to the most appropriate IMCA available in your area, who will be in contact within 5 working days (or sooner if the case is urgent).  Hampshire IMCA enquiries to 01256 478999.

Referrals for a DOLS IMCA in Hampshire

All referrals for a DOLS IMCA will come from the Supervisory Body when an application for a Standard Authorisation is made – residential care homes or hospitals do not need to refer cases to us directly.
